Handbook

Any suggestions on how to chart a progress report on my players for their professors to sign to keep me informed that all players are attending class on a weekly basis?? The players will be required to bring it to class and have each professor initial, or sign at the end of each week.

How could I word it to the professors so they understand the point and not the athlete that will be handing them a piece of paper asking for their signature!!
